What happens when you go for an appointment?
At a first appointment, the therapist will need to collect your full contact details during a consultation, so that she will be able to contact you in future to remind you of next appointment times or to cancel in case of a need to reschedule, or due to unforeseen circumstances. It may also be used (e.g. email) as a means in future to inform you of news & offers, any upcoming local events being organised, ways the therapist may be of assistance, and videos and services. You can OPT-IN by providing your email address, and you can OPT-OUT at any time by letting her know you wish to be removed from the mailing list with immediate effect.
During this first consultation, it will also be necessary to obtain more detailed information on your medical history and present health problems/issues, so that the treatment may be tailored to your needs to carry out the safest and most beneficial session for you.
The personal and strictly private information you provide, will be paperbased and kept in a locked filing cabinet in the treatment room, to which only the therapist has a key. It will be updated regularly, together with a client treatment record and a running commentary of any medical changes for each client.
As a client, you promise to keep the therapist updated on your health, should there be any changes to previous answers given.
No information about any client that has been confidentially disclosed to the therapist, will be discussed or shared with any third party without written consent of the client or parent/guardian if the client is under 18.
